To initiate the DNA Testing process:

Please send us the following case setup information:

1. Petitioner:  name, address in USA, phone# and email;
2. Test Participants: names, date-of-birth, role (e.g. mother, child, father, etc), city/county of residence;
3. If a Test Participant lives abroad:  include the name and phone # of the contact in their country of residence;
4. If a Test Participant is a minor: indicate the name of the parent or legal representative who will be consenting for the test;
5. Email address(es) authorized to receive test results;
6. Copy of the letter requesting the DNA test, if one is available.  Email it to info@intelligenetics.com
