Once you complete the form, the following “Checkout” options are available: i) complete the transaction with secure credit/debit processing system SQUARE; ii) select a payment plan that allows you to initiate the process with a minimum payment of $300; or ii) submit the form without payment if you wish to request other method of payment (e.g. cash, check, money order, Western Union, MoneyGram).

All results for DNA processing are legally admissible (i.e., certified by our AABB-Accredited Laboratory).
